Understanding the Decision

Run a gate, read the decision, and know exactly what it is based on.

Now the moment of truth. What the decision means, and what it never quietly does.

Step 1

Every decision is kept, versioned, and locked, with its confidence, the person who decided, and the date. Nothing in Decision History can be quietly rewritten later.

Step 2

A REFINE decision is not a failure. It names what still needs correcting, writes out the required actions, and holds the work until the gate is re-run. The jurisdiction pack applied to the decision is shown at the top. In this demo, the UK pack, with the HSE as the named regulator.

Step 3

A PASS is also precise: all assessed controls confirm readiness, and the work still does not proceed until the responsible person and competent supervisor have reviewed and authorised the gate. The system never grants permission on its own.
