How Decisions Are Made

WorkGateIQ Decision Engine — Transparency Statement

The short version: WorkGateIQ does not make safety decisions. You do. The system structures, verifies, and records the inputs that a named, competent, responsible person reviews — and then reflects their decision back as a clear, auditable outcome. The engine does not replace human judgment. It protects it.

The Full Maintenance-to-Permission Chain

WorkGateIQ covers the full chain — from the first maintenance request to the final closeout record. Every step keeps evidence, accountability, and human decision authority visible.

1. Request

Capture the asset, issue, discipline, and priority.

2. Plan

Define scope, people, method, permits, isolation, and Safety Gate need.

3. Verify Readiness

Confirm evidence, competence, emergency readiness, and Stop Work authority.

4. Release to Work

Authorised human release; the system verifies and blocks.

5. Execute & Capture Evidence

Checklists, readings, photos, notes, parts, and Stop Work events.

6. Inspect & Manage Exceptions

Test results, failed checks, corrective actions, deferments, escalations.

7. Hand Back & Close

Authorised handback, equipment availability marking, locked record.

8. Retrieve Intelligence

Evidence pack, closeout report, asset history, audit search.

Decision-Support Only

WorkGateIQ supports pre-work safety readiness decisions. It does not replace certified safety professionals, competent supervisors, legal advisors, regulators, or your organisation's safety management system.

PASS

PASS means the required task information, accountability, PTW status, critical safeguards, and emergency readiness have been confirmed based on the current gate inputs.

A PASS outcome means: "The pre-work record is complete and verified as of the time of submission." It does not mean the work is unconditionally safe. The named responsible person retains full authority and accountability on site at all times.

REFINE

REFINE means the work may become ready, but some safeguards, documents, evidence, or accountability details need clarification before the task should proceed.

The system identifies exactly which items need attention — no guesswork required. Address flagged items and run a new gate.

FAIL

FAIL means the work is not ready to proceed because critical information, valid permits, emergency readiness, or critical safeguards are missing.

Work should not proceed until FAIL conditions are resolved and a new gate is created.

What the Engine Does NOT Do
  • Authorise work on behalf of your organisation
  • Override the authority of a competent supervisor or PTW issuer
  • Use machine learning or predictive models to estimate risk
  • Make probabilistic judgments — outputs are based entirely on what your team confirms
  • Replace your organisation's statutory safety management system
  • Account for on-site conditions that cannot be captured in a digital record
Human Accountability

WorkGateIQ does not authorise work on its own. A named responsible person and competent supervisor remain accountable for reviewing the gate decision and confirming readiness.

Decision Records

Every gate decision creates a decision record with status, risk level, confidence level, responsible person, competent supervisor, timestamp, and decision version.

Decision status (PASS / REFINE / FAIL)
Inherent & residual risk level
Confidence level
Responsible person
Competent supervisor
Precise timestamp (UTC)
Decision version
Locked — cannot be silently altered
Alignment With UK HSE and ISO 45001

WorkGateIQ's gate structure is designed to support compliance with principles found in ISO 45001 operational planning and control and HSE permit-to-work guidance.

WorkGateIQ does not certify your organisation as ISO 45001 compliant. It is designed to support and evidence your operational control processes within a broader safety management system.

"No work should proceed until minimum safeguards, accountability, permits, and emergency readiness are confirmed — by a real person who takes responsibility for that decision."
